Contested Parking Ticket Form

If you would like to contest a parking ticket that was issued by a City of Port Washington Police Officer, you may do so by completing and submitting this form. The form will be submitted to a Port Washington Police Department shift supervisor. Following review by the department, you will be notified by phone or if you request, by e-mail as to the outcome of the review. Please allow 3 days for a response. If you do not receive a response within 3 days, you may call the Port Washington Police Department at (262) 284-2611.
